How do I pay for treatment?

Mindful Physical Therapy is in network with and will directly bill Medicare and Medicare Secondary Policies, Anthem Blue Cross, and Blue Shield. To self pay, the fee is $150 for a one-hour Examination and Treatment, and $95 for 45-minute follow-up visits. If you self pay, we will provide you with a “Superbill” which includes your diagnosis, treatment codes, and record of payment. This can be submitted to insurers that we are not in network with, and for health savings account.

Do I need a referral?

You do not need a doctor’s prescription to see a physical therapist in California. You can be treated by a physical therapist for 12 sessions or 45 calendar days, whichever occurs first. After this, you will need a prescription by a physician, osteopath or podiatrist certified in the state of California. Some health insurance companies still do require a physicians referral for reimbursement. To ensure coverage and minimize your out-of-pocket cost, please bring a recent prescription from your doctor indicating:

  1. Date of prescription
  2. Number of visits requested
  3. Your diagnosis
  4. Legible doctor’s name (print it next to name if signature is not legible)

How long is my appointment?

Your initial evaluation will be an hour long and subsequent follow-up appointments are 45 minutes.

What is your cancellation policy?

We require 24 hours notice for cancellations in order to avoid a $45 cancellation or no show fee.

What do I need to bring to my first appointment?

  1. Insurance card (or copy)
  2. Prescription

How should I dress?

Wear comfortable exercise clothing and tennis shoes. Wear clothing that makes your injury accessible. For example, if you have a knee injury, wear shorts or pants that are loose enough where we can easily access the specific part of the body that is being worked on.
